We are honored to walk with you during this time of loss. To ensure we can serve your family with care and clarity, please carefully review the instructions below before submitting the Funeral Request Form.

1. Notify the Church First

If you have not already, please inform your assigned Deacon/Deaconess or our Administrative Assistant about your loved one’s passing.

2. Complete the Funeral Request Form

Complete this Funeral Request Form as soon as possible. This form is required to move forward with planning and to gather all the necessary information.

Very Important:

Submitting this form does NOT guarantee your requested date. Final confirmation depends on church calendar availability and ministry coordination.

Please Do NOT share any funeral service details publicly (on social media, in obituaries, or flyers) until you receive official confirmation from the church office.

3. What Happens After You Submit the Form

  • The completed form will be sent directly to our Administrative Assistant, who will coordinate with the relevant ministry departments to confirm availability or arrange appropriate coverage.

  • Your family’s contact information will be shared with the relevant ministries so they can follow up if needed.
